I would like to ask your opinion about Magnesium Sulfate for the Prevention of Cerebral Palsy in preterm deliveries. NNT: 63?. Is it worth it?, what dosage? why is it useful in preterms but not in terms?
I think the dosage tested in the studies would be most reasonable to use. The SOGC in Canada recommend 4 g as a loading dose and then 1 g per hour until birth, you can find their really nice guideline here. It is true that the NNT is large, especially for the more mature babies, so it is really a matter of judgement for a baby more than 30 weeks.
It might work in term babies as well, we just don’t know, but the overall incidence in term babies is 2 per 1000, so we will never be able to find out if it works or not, unless we can find a higher risk group, and then study them.
